  1. If you provide care and support to a loved one, a carer's assessment can help you identify the impact this has on you and what things might help to make your life easier.
    
    The individual(s) that you care for must be an adult and live within the Stockton-on-Tees Borough.
    
    You and the person you care for must be at least 18 years old to complete this form.
  2. This form will ask questions about:
