Apply for a sponsored day care review

Use this service to:
1. ● apply for a review to extend existing sponsored day care when the original placement is coming to an end
  
  Application takes around 30 minutes to complete.
What you'll need
1. ● to be a professional involved with the child you are referring
  
  ● authorisation from the applicable parent/carer/young person to apply
  
  ● details of the child's health visitor
