• ● marriage certificate or change of name deed if you are changing your name

This form should be completed by the designated premises supervisor or premise licence holder.

The fee for an updated licence is £10.50

You are required to return your old licence to Licensing Service, Municipal Buildings, Church Road, Stockton-on-Tees, TS18 1LD
