  ● opt out of contributing to the No Limits giving scheme

  ● your Directorate details

Before you start
1. A one off payment will be deducted from salary in month following receipt of form.
  
  Yearly payments will be deducted from your April salary.
  
  Monthly payments will be deducted from your monthly salary.
What happens to my information?
