Referral for Home Improvement Agency support

● access the hospital discharge service to help you return home after a hospital stay with minor adaptations

● access the adaptations service to install minor adaptations (grabrails) and sensory loss equipment around the home to ensure safe and independent living

● access the handyman service to provide low level DIY type jobs that will reduce accidents, falls and unnecessary hospital admissions

● access the garden assistance scheme to provide low level works to reduce accidents and promote mental health and wellbeing

Qualifying criteria applies.

Referral takes around 10 minutes.
