No child under 13 years of age shall be employed.
The parent or guardian of the child must also complete and submit the child health questionnaire before a licence can be granted.
Both forms must be submitted within 10 days of employment.

This information is collected to process licence requests in accordance with the Children and Young Persons Act 1933 to 1963 (opens in new tab) as amended by the Education Acts 1944 to 1976 (opens in new tab) and the Children Act 1972 (opens in new tab). Also in accordance with the Stockton-on-Tees Byelaws relating to the Employment of children dated 8th february 1999.
The information you provide on this form will be held by Stockton-On-Tees Council and used by us to process your request. The data is held in line with the Council’s Retention schedule and then destroyed in a secure manner.
Stockton-on-Tees Borough Council use the personal information provided in order to deal with your request. The data may be shared internally and/or externally, including with schools, school health and child employment officers in other councils, as well as the employer and parent or guardian of the child to assess the suitability of the child to the employment proposed.
