Notify us of a change of name or address for your personal licence

Completion takes around 5 minutes.

● a debit or credit card to pay for your new licence

● a digital passport style photo that can be uploaded

This form should be completed by the licence holder

The fee for an updated licence is £10.50

You are required to return your licence to the licensing office
