This form allows colleagues who don't have access to a work computer to easily submit nominations using another device. It can also be used by managers to nominate non-desk-based colleagues

  ● if you are nominating through the STARS recognition scheme, details of why they deserve to be recognised for their achievements
  
  ● if you are nominating through the Annual Colleague Awards, you'll need to explain how they went above and beyond to meet the criteria of the award
  
  ● if you have any relevant evidence to support an Annual Colleague Award, you can upload it with your nomination

1. All nominations for the Annual Colleague Awards will be reviewed by a judging panel to be shortlisted. Finalists will be invited to the awards ceremony which will take place at Preston Park Museum and Grounds on 19 November 2026.
  
  The closing date for all Annual Colleague Award nominations is 14 September 2026.
  
  Nominations for the STARS recognition scheme will be coordinated by the People, Wellbeing and Engagement team. All nominees will receive an invitation to attend a coffee and cake thank you session with Mike Greene.
