  ● authorisation from the applicable parent/carer/young person to apply
  
  ● details of the child's health visitor

1. You will need to complete all 3 sections of the form to submit your application, you can save your progress throughout.
  
  Only providers with a Good or Outstanding rating from Ofsted can be approved, nursery or childminders ratings can be found at Stockton information directory (opens in new tab).
